Please note that Revenue has updated the Enduring Certificate (formerly CHY3) and Annual Certificate (formerly CHY4). This has been done to provide further information about how charities can claim tax relief on donations.
The updated versions of these forms are now available on the Revenue website at the following links:

and should be used with immediate effect by all charities / entities operating the Charitable Donation Scheme.
Note: The Charitable Donation Scheme allows for the repayment of tax relief on qualifying donations made to approved bodies. If you are not submitting claims under the scheme please contact the Charities and Sports Exemption Unit to cease your registration.
